Woodhaven Application of Services
Woodhaven programs have been developed to provide individualized supports for adults with developmental disabilities or neurodiversity. All Woodhaven program services are funded through a MO Department of Mental Health Medicaid Waiver or private pay. Apply
All applications will be reviewed for eligibility. Eligibility for services will be based upon qualifying diagnosis, prior history, and available opening in the chosen program. A representative of Woodhaven will contact you after reviewing the application. If there is not a current opening available in a program, placement on a waiting list is available. If accepted, Woodhaven will work closely with the applicant’s Service Coordinator to finalize approval with the MO Dept of Mental Health. There will be no guaranteed timeline from the date of approval for services to the actual start of those services. Many factors can affect this timeframe, such as final budget approval, staffing availability, or submission of all necessary documentation prior to start. Once all necessary steps have been completed, Woodhaven will approve an official start date to begin the implementation of services.
